Replace a Lost, Stolen, or Mutilated Vermont Driver's License or ID Card

If your Vermont driver's license or identification card has been lost, stolen, or mutilated, you can apply for a replacement or duplicate license at your local Department of Motor Vehicles (DMV) Office.

Instructions for replacing a lost, stolen or mutilated Vermont driver's license or identification card:

If you have a license or identification card with a photo taken after January 5, 2004 you can apply for a duplicate by mail, following the same instructions as given for out of state residents. To apply in person, follow the instructions below:

  1. Locate the Vermont DMV Office nearest you. Be sure to note the days and hours of operation.
  2. Download and complete Application for a Duplicate License.
  3. Bring proper identification. The state of Vermont requires that you present one piece of primary ID and one piece of secondary ID when applying for a duplicate driver's license or identification card.
  4. Bring the correct fee. Normally, there is a $15 fee for replacing a lost, stolen, or mutilated license. Payment can be made by cash, checks or credit cards. Please contact your local DMV office to confirm the fee before applying in person, since fees may vary.

Instructions for Vermont residents who are temporarily out of state:

Vermont residents who are temporarily out of state can apply for a replacement of a lost license by mail if a recent photo of the applicant (taken Jan.5, 2004 or later) is on file with the DMV.

  1. Download and complete the application form from the DMV website.
  2. Obtain a check or money order in the correct amount, made payable to: Department of Motor Vehicles. Please contact the DMV at (802) 828 2000 to confirm the correct fee, as fees may vary.
  3. Mail the completed form along with two photocopied documents of identity and the payment to:

Additional Information

  • If your license or identification will expire in 30 days, you can renew instead of applying for a replacement.
  • You cannot apply for a replacement driver's license or identification online.
