Replace a Lost, Stolen, or Mutilated Iowa Driver's License or ID Card

If your Iowa driver's license or identification card has been lost, stolen, or mutilated, you can apply for a replacement or duplicate license at your local Motor Vehicle Division (MVD) driver's license station.

Instructions for replacing a lost, stolen or mutilated Iowa driver's license or identification card:

  1. Locate the Iowa MVD driver's license station Iowa Driver's License Issuance Site. Be sure to note the days and hours of operation.
  2. Bring appropriate identification. The state of Iowa requires that you present one piece of primary ID and one piece of secondary ID when applying for a duplicate driver's license or identification card. Persons under the age of 18 must present one piece of primary ID, one piece of secondary ID, proof of Social Security number and a parent's consent form.
  3. Bring the correct fee. Normally, there is a $3 fee for replacing a lost, stolen, or mutilated license. Please contact your nearest you to confirm the fee before applying in person, since fees may vary.

Instructions for Iowa residents who are temporarily out of state:

Iowa residents who are temporarily out of state and unable to apply in person may apply for a duplicate license or identification card by calling the Office of Driver Services at (800) 532 1121. A temporary paper license will be mailed to you which can be used till you are able to visit an Iowa driver's license station in person to apply for a duplicate.

Additional Information

  • If your license or identification will expire within the next 30 days, you can renew your license or identification card instead of applying for a duplicate.
  • You cannot apply for a replacement driver's license or identification online.
