Replace a Lost, Stolen, or Mutilated Iowa Driver's License or ID Card
If your Iowa driver's license or identification card has been lost, stolen, or mutilated, you can apply for a replacement or duplicate license at your local Motor Vehicle Division (MVD) driver's license station.
Instructions for replacing a lost, stolen or mutilated Iowa driver's license or identification card:
- Locate the Iowa MVD driver's license station Iowa Driver's License Issuance Site. Be sure to note the days and hours of operation.
- Bring appropriate identification. The state of Iowa requires that you present one piece of primary ID and one piece of secondary ID when applying for a duplicate driver's license or identification card. Persons under the age of 18 must present one piece of primary ID, one piece of secondary ID, proof of Social Security number and a parent's consent form.
- Bring the correct fee. Normally, there is a $3 fee for replacing a lost, stolen, or mutilated license. Please contact your nearest you to confirm the fee before applying in person, since fees may vary.
Instructions for Iowa residents who are temporarily out of state:
Iowa residents who are temporarily out of state and unable to apply in person may apply for a duplicate license or identification card by calling the Office of Driver Services at (800) 532 1121. A temporary paper license will be mailed to you which can be used till you are able to visit an Iowa driver's license station in person to apply for a duplicate.Additional Information
- If your license or identification will expire within the next 30 days, you can renew your license or identification card instead of applying for a duplicate.
- You cannot apply for a replacement driver's license or identification online.
