If your Maine driver's license or identification card has been lost, stolen, or mutilated, you can apply for a replacement or duplicate license at your local Bureau of Motor Vehicles (BMV).
Instructions for replacing a lost, stolen or mutilated Maine driver's license or identification card:
You can apply for a duplicate license online, by mail or in person. An application for a duplicate can be made online at the BMV website if your original license was a digital license. To apply by mail follow the same instructions as for an out-of-state resident. To apply in person, follow the instructions below.
- Locate the Maine BMV office nearest you. Be sure to note the days and hours of operation.
- Bring proper identification. The state of Maine requires that you present two pieces of ID, one showing date of birth and the other having a written signature. Proof of Maine residency also needs to be shown. If the original was a photo license or identification card, the image must be retaken.
- Complete and bring a request for duplicate license form.
- Bring the correct fee. Normally, there is a $5 fee for replacing a lost, stolen, or mutilated license. Please contact your local BMV office to confirm the fee before applying in person, since fees may vary.
Instructions for Maine residents who are temporarily out of state:
Maine residents who are temporarily out of state or unable to apply in person may apply for a duplicate license or identification card through the mail. Such persons may include military personnel or students who are either out of the state or country, among others.- Download and complete the application form from the BMV website.
- Obtain a certified check, cashier's check or money order in the correct amount, made payable to: Bureau of Motor Vehicles. Please contact the BMV at 207-624-9000 to confirm the correct fee, as fees may vary.
- Mail the completed form, along with your payment to:

Additional Information
- If your license or identification will expire within the next 30 days, you can renew your license or identification card instead of applying for a replacement.
- You cannot apply for a replacement driver's license or identification online if you do not have a digital license, if your photo was taken when you were under 23 years of age or for a commercial driver's license.
