Replace a Lost, Stolen, or Mutilated Idaho Driver's License or ID Card
If your Idaho driver's license or identification card has been lost, stolen, or mutilated, you can apply for a replacement or duplicate license at your local Idaho Division of Motor Vehicles (DMV) driver's license office.
Instructions for replacing a lost, stolen or mutilated Idaho driver's license or identification card:
- Locate the DMV driver's license office nearest you. Be sure to note the days and hours of operation.
- Bring proper identification. The state of Idaho requires that you present a primary ID document OR a combination of primary and secondary ID documents when applying for a duplicate driver's license or identification card.
Persons under the age of 18 must be accompanied by a parent or legal guardian with a picture ID. They must also provide proof of enrollment or graduation from a recognized high school or equivalent institution.
- Bring the correct fee. Normally, there is an $11.50 fee for replacing a lost, stolen, or mutilated license. Please contact your local DMV driver's license office to confirm the fee before applying in person, since fees may vary.
Instructions for Idaho residents who are temporarily out of state:
Idaho residents who are temporarily out of state and unable to apply in person may apply for a duplicate license or identification card through the mail or fax. Such persons may include military personnel or students who are either out of the state or country, among others.
- Download and complete the application form from the Division of Motor Vehicles website.
- Obtain a certified check, cashier's check, or money order or provide Mastercard or Visa card information in the correct amount, made payable to: Idaho Transportation Department. Additional Information
- If your license or identification will expire within the next one year, you can renew your license or identification card instead of applying for replacement.
- You cannot apply for a replacement driver's license or identification online.
