Replace a Lost, Stolen, or Mutilated Arizona Driver's License or ID Card
If your Arizona driver's license or identification card has been lost, stolen, or mutilated, you can apply for a replacement or duplicate license in person at your local Motor Vehicle Division (MVD) office, online or on the phone.

You can obtain a duplicate Arizona driver's license online at this website or over the phone by calling (877) 301 8093 during regular business hours.
To apply in person, follow the instructions below.
- Locate the Arizona MVD office nearest you. Be sure to note the days and hours of operation.
- Bring proper identification. The state of Arizona requires that you present one piece of primary ID and one piece of secondary ID, with at least one of them being a photo ID, when applying for a duplicate driver's license or identification card. Alternatively, you can bring three pieces of non-photo ID.
- Bring the correct fee. Normally, there is a $4 fee for replacing a lost, stolen, or mutilated license. Please contact your local MVD office to confirm the fee before applying in person, since fees may vary.

Arizona residents who are temporarily out of state may apply for a duplicate license or identification card through the phone by calling (877) 301 8093. A duplicate license will usually be dispatched on the same day.Additional Information
