Replace a Lost, Stolen, or Mutilated Alaska Driver's License or ID Card

If your Alaska driver's license or identification card has been lost, stolen, or mutilated, you can apply for a replacement or duplicate license at your local Division of Motor Vehicles (DMV) office.

Instructions for replacing a lost, stolen or mutilated Alaska driver's license or identification card:

  1. Locate the Alaska DMV office nearest you. Be sure to note the days and hours of operation.
  2. Bring proof of identification. The state of Alaska requires that you present one piece of primary ID and one piece of secondary ID when applying for a duplicate driver's license or identification card.
  3. Bring the correct here. Normally, there is a $15 cash fee for replacing a lost, stolen, or mutilated license. Non-personalized checks are not accepted. Please contact your local DMV office to confirm the fee before applying in person, since fees may vary.
  4. At the DMV office, you will have to take a vision test and also have a new photo of yourself taken.

Instructions for Alaska residents who are temporarily out of state:

Alaska residents who are temporarily out of state and unable to apply in person may apply for a temporary license by fax or mail.
  1. Download and complete the application form.
  2. Obtain a check or money order for $5.00, made payable to: State of Alaska or DMV. If submitting by fax, credit card information can be provided in the application form. Please contact the DMV at 1-907-269-5551 to confirm the correct fee, as fees may vary.
  3. Fax the completed form to (907) 465-5509 or mail it along with your payment to:

  1. After returning to Alaska, you will have to visit a DMV office to obtain a proper duplicate license.

Additional Information

  • If your license or identification will expire within the next one year, you can renew your license or identification card instead of applying for a replacement.
  • You cannot apply for a replacement driver's license or identification online.
