How to Replace a Commercial Driver's License (CDL) in Colorado

If your Colorado commercial driver's license (CDL) is lost, stolen, or destroyed, you can apply for a replacement license. While not required by Colorado law, it is a good idea to notify your local police department if you believe your CDL has been stolen.

APPLYING FOR A DUPLICATE CDL

  1. You will need to visit your local driver license office in person in order to apply for a replacement CDL. If your CDL was stolen, you may file a police report and attach it along with an Affidavit of License Theft Form to apply for the duplicate CDL.
  2. You must present one valid form of proof of identification along with Colorado address proof and birth date proof.
  3. Submit the replacement fee of $7.50 (only if it is the first replacement) or $14 (if it is a second replacement or higher).
  4. If your current CDL is damaged, you must surrender it at the driver license office when you apply for a replacement.
