How to Replace a Commercial Driver's License (CDL) in Illinois
If your Illinois commercial driver's license (CDL) is lost, stolen, or destroyed, you can apply for a replacement license. You should notify your local police department as early as possible to inform about your lost or stolen CDL.
APPLYING FOR A DUPLICATE CDL IN PERSON
- You will need to visit your local Secretary Of State VSD Office in person in order to apply for a replacement CDL.
- You must present two valid forms of identification, one document for signature proof and one document for either social security number proof, or birth date proof or Illinois address proof. All documents must be presented in original since photocopies are not accepted.
- Submit the replacement fee of $5. Check the accepted form of payment with your local Secretary Of State VSD Office before you visit. If you are 60 years old or older and have a copy of the police report you filed for your lost/stolen CDL, the replacement fee will be waived.
- If your current CDL is damaged, you must surrender it at the Secretary Of State VSD Office when you apply for a replacement.
APPLYING FOR A DUPLICATE CDL BY MAIL
You may be eligible to receive a duplicate CDL by mail when you are out of Illinois. You can call the License & Medical Review Section at (217) 782-2720 or your driver license to find out how to receive a temporary license or to request an application for a duplicate to be sent to you at your address outside Illinois.ADDITIONAL INFORMATION
- You cannot apply for a replacement commercial driver's license online.
- If your name or address has changed, you may need to apply for a replacement CDL in order to correct the information.
