How to Replace a Commercial Driver's License (CDL) in New York
If your New York commercial driver's license (CDL) is lost, stolen, or destroyed, you can apply for a replacement license. While not required by New York law, it is a good idea to notify your local police department if you believe your CDL has been stolen. You can apply for a replacement CDL in person, online and by mail.
APPLYING FOR A DUPLICATE CDL IN PERSON
- Visit your local DMV office in order to apply for a replacement CDL in person.
- You must present valid forms of identification and a completed replacement application form (MV-44).
- Submit the replacement fee of $15, payable by cash, check or credit card. Your replacement fee may be waived if you get form MV-78B (only available from the police) completed by a police agency to show that your license was stolen.
- If your current CDL is damaged, you must surrender it at the DMV office when you apply for a replacement.
APPLYING FOR A DUPLICATE CDL ONLINE
- You may replace your stolen/lost CDL online, unless your driving privileges are suspended/revoked or if you do not meet the eligibility criteria.
- You will need your Client ID Number or Name as stated on your previous license.
- You will also need to enter your date of birth, Social Security number and zip code.
- Submit a valid e-mail address where you will receive confirmation of replacement order.
- Use a Credit card (Visa, American Express, MasterCard or Discover) to make the payment of $15 for the replacement.
Note: It can take up to four weeks for you to receive your duplicate CDL by mail.
APPLYING FOR A DUPLICATE CDL BY MAIL
- Complete the replacement application form (MV-44).
- Attach clear photocopies of proof of identification.
- Include a money order or check for the replacement fee of $15.
- Mail the package either to:

Note: The duplicate CDL will arrive by mail within four weeks. If you wish to receive the duplicate license faster, include a pre-paid, pre-addressed express mail package with your application.
ADDITIONAL INFORMATION
- The original form MV-78B must be completed by a police agency, if the license was stolen, to waive the replacement fee of $15. This form cannot be used when applying for a duplicate CDL online or by mail. It can only be used when the application is made in person at a DMV office.
- Although you can apply for a replacement commercial driver's license online or through the mail, it can take up to four weeks to receive it in the mail. Apply in person if you need your CDL earlier.
- If your name or address has changed, you may need to apply for a replacement CDL in order to correct the information.
