How to Replace a Commercial Driver's License (CDL) in Idaho

If your Idaho commercial driver's license (CDL) is lost, stolen, or destroyed, you can apply for a replacement license. While not required by Idaho law, it is a good idea to notify your local police department if you believe your CDL has been stolen.

APPLYING FOR A DUPLICATE CDL IN PERSON

  1. You will need to visit your local Driver Licensing Office in person in order to apply for a replacement CDL.
  2. You must present valid forms of identification showing age, name and picture proof along with an Idaho address proof.
  3. Submit the replacement fee of $11.50.
  4. If your current CDL is damaged, you must surrender it at the Driver Licensing Office when you apply for a replacement.

APPLYING FOR A DUPLICATE CDL BY MAIL

You may be eligible to receive a duplicate CDL by mail when you are out of Idaho. You can complete the Online Duplicate Driver's License Form, enclose the replacement fee of $11.50 in the form of a check or money order; or include your credit card (MasterCard/Visa) information and mail it to:
Idaho Transportation Department
Driver's License
PO Box 7129
Boise ID 83707-1129
If you choose to pay by credit card, you may also fax the application to (208) 334-8586.
