How to Replace a Commercial Driver's License (CDL) in Hawaii
If your Hawaii commercial driver's license (CDL) is lost, stolen, or destroyed, you can apply for a replacement license. While not required by Hawaii law, it is a good idea to notify your local police department if you believe your CDL has been stolen.
APPLYING FOR A DUPLICATE CDL
- You will need to visit your local CDL unit or service center, which differ by county, in person in order to apply for a replacement CDL. These locations are:
Honolulu County:

- You must fill out the application form and present valid forms of identification and other proofs, as required by each county. Hawaii County requires a new photograph; Kauai requires proof of name, birth date, social security card and if applicable, proof of name change; Honolulu and Maui counties only require proof of identification.
- Submit the appropriate replacement fee, which differs by county. For Honolulu County it is $5, for Hawaii County it is $15, for Kauai County it is $5, and for Maui County, the fee is set as per the county's annual budget.
- If your current CDL is damaged, you must surrender it at the CDL unit or service center when you apply for a replacement.
ADDITIONAL INFORMATION
- You cannot apply for a replacement commercial driver's license online or through the mail.
- If your name or address has changed, you may need to apply for a replacement CDL in order to correct the information.
