How to Replace a Lost Vehicle Title in Hawaii

You may apply for a duplicate vehicle title if your original title is lost, stolen, or damaged. A vehicle with a missing title cannot be bought or sold, so owners must apply for and obtain a replacement before they can sell their vehicle.

WHAT YOU NEED WHEN APPLYING FOR A DUPLICATE/REPLACEMENT TITLE

IN THE CITY AND COUNTY OF HAWAII:

  1. Complete the Application for Duplicate Motor Vehicle Certificate of Title.
  2. Once completed, you must have the application form properly notarized.
  3. If your original title is damaged or mutilated, you must return the original title when applying for a replacement.
  4. If your title shows a lienholder, you must include the name of the lienholder and his/her signature on the form as well.

FEES

Along with your application form and the other materials, you must submit a $5.00 duplicate title fee. Checks and money orders should be payable to the Hawaii Department of Finance.

SUBMITTING THE APPLICATION

You can submit the application and fees for a duplicate title at your local contract office, or mail the package to:

IN THE CITY AND COUNTY OF HONOLULU:

  1. Complete the Application for Duplicate Motor Vehicle Certificate of Title.
  2. Once completed, you must have the application form properly notarized.
  3. If your original title is damaged or mutilated, you must return the original title when applying for a replacement.
  4. If your title shows a lienholder, you must include the name of the lienholder and his/her signature on the form as well.

FEES

Along with your application form and the other materials, you must submit a $10.00 duplicate title fee. Checks and money orders should be payable to the City and County of Honolulu.

SUBMITTING THE APPLICATION

You can submit the application and fees for a duplicate title at your local contract office, or mail the package to:

IN THE CITY AND COUNTY OF MAUI:

  1. There are no online forms available so you will have to visit the local Islands of Maui to collect one.
  2. Once completed, you must have the application form properly notarized.
  3. If your original title is damaged or mutilated, you must return the original title when applying for a replacement.
  4. If your title shows a lienholder, you must include the name of the lienholder and his/her signature on the form as well.
